Include a copy of original patient requisition when sending sample. Send whole blood on ice packs, but sample must be protected from freezing at any time or sample will be rejected. Specimen must be received at HSC-Hematology MS5 within 72 hours of collection, and not later than 11:00 a.m. on Fridays.
