Test Indications: 

Aids in the evaluation of rheumatoid arthritis.  Use of the anti-CCP assay in association with RF assay maximizes the sensitivity/specificity ratio. It must not be forgotten that 15-20% of RAs are positive for RF but negative for anti-CCP and that about half of the RAs negative for RF are anti-CCP positive. Simultaneous positivity for RF and CCP has a positive predictive value of 100%.

Serial monitoring of RA patients who are positive for anti-CCP is not appropriate as these autoantibodies do not correlate with clinical course or with response to drug treatment.

 A negative result does not necessarily exclude autoimmune disease.  Positive results should not be interpreted in isolation and must be correlated with relevant clinical findings.



Method of Analysis: The IDS CCP assay is based on chemiluminescence technology. Patient serum is incubated with magnetic particles coated with the specific antigen.  Following the washing step after the first incubation, an anti-human IgG antibody labelled with acridinium is added, followed by a subsequent incubation step. The magnetic particles are captured using a magnet and a wash step performed to remove any unbound analyte. Trigger reagents are added; the resulting light emitted by the acridinium label is directly proportional to the concentration of anti-CCP IgG in the sample.
