Clinical Information:
Testing is performed at the Health Sciences Centre Immunology Laboratory.
 
Urine proteins are separated in an electric field according to their size, shape, and electric charge.  A pentavalent antisera is applied to the gel forming insoluble complexes with any immunoglobulins present.  A characteristic monoclonal band (M-spike) is often found in the urine of patients with monoclonal gammopathies such as multiple myeloma, primary amyloidosis, and light chain deposition disease. The initial identification of an M-spike or an area of restricted migration is followed by immunofixation to identify the immunoglobulin heavy chain and/or light chain with application of gamma, alpha, mu, kappa, and lambda antisera.
           
When performed at screening, Serum Free Light Chains can replace the 24hr urine protein IFE for all diagnoses except amyloidosis.  However, once a diagnosis of monoclonal gammopathy is made, the 24hr urine protein IFE should be done.
 
Results should be used in conjunction with other laboratory and clinical findings.

Refrigerate specimen during collection and send refrigerated.

Aliquot specimen to an appropriately labelled 60 mL urine bottle.
Refrigerate specimen during collection and send refrigerated.
Measure and record volume on requisition.
Total protein values are required before sending to HSC for electrophoresis for those sites capable of performing the test.
