All requisitions require a physician's name with signature and collection time and date

Test Indications: Place tissue specimen immediately into glass specimen vial containing 2.5% buffered glutaraldehyde.  Glass vials containing 2.5% buffered glutaraldehyde can be obtained from pathology.  Within HSC, call EM department to replenish stock. 
Specimen should be refrigerated until sent to pathology.
 
Transportation: For sites outside of Winnipeg, please contact HSC EM Lab to arrange for transportation of specimen.

Specimen Required:

Adult: Tissue sample must be no larger than 2 mm.  When placed into glutaraldehyde container, the solution must cover the tissue.
Pediatric: Tissue sample must be no larger than 2 mm.  When placed into glutaraldehyde container, the solution must cover the tissue

Specimen Handling: All specimens must be properly labeled with appropriate patient identification.  All specimens must be accompanied by a completed pathology requisition.  Pertinent information including patient demographics, clinical history, physician’s name and specimen type is required as per Shared Health Diagnostic Services Specimen Acceptance Policy 10-50-03
