Alternate Name(s): Screening Procedure for Candida auris.
Test Indications: The intent of this test is to determine if a patient is colonized with Candida auris. Candida auris is a highly drug-resistant Candida species that is associated with healthcare-associated infections and worldwide outbreaks. Most Candida auris isolates have been reported to be resistant to fluconazole. Most Candida auris isolates identified in North America are susceptible to echinocandins and amphotericin B; however, rare isolates resistant to both drugs has been reported. Test Approval Requirements: Ordering restricted to Requests for testing must satisfy requirements set out in the Infection Prevention & Control Manual (WRHA) or the Guidelines for the Prevention and Control of Antimicrobial-Resistant Organisms (Manitoba Health, Seniors and Active Living). Recommendations: Do not screen patients known to be CPE positive. For CPE screening criteria please refer to the Winnipeg Regional Health Authority – Acute Care Infection Prevention & Control Manual and/or Manitoba Health, Seniors and Active Living – Guidelines for the Prevention and Control of Antimicrobial-Resistant Organisms A negative Candida auris culture result does not rule out the presence of Candida auris. Adult and Pediatric: 1 swab Collection Information: • Do not routinely screen patients for Candida auris on admission. • Obtain test approval by IP&C practitioner or the microbiologist. • After test is approved notify the Microbiology Laboratory in advance of screening. • Swab both axillae: Rub firmly over skin folds; using the same swab, swab both groin creases. (For other body sites use one swab per site. Do not collect specimens from closed wounds/ lesions/ incisions). • Place swab in transport container • Label the container with the site of sample collected and at least two unique patient identifiers. • ensure the specimen is accompanied by the appropriate requisition that has been completed with all pertinent patient information. Under section “Other Tests” of the microbiology requisition, indicate specimen, site, specify test as “Candida auris screen” and rationale for the request. Shipping & Storage: Specimen must be delivered to the laboratory as soon as possible; ideally within 24 hrs of collection.
Local: ≤2 hrs, store at room temperature. Courier/local storage: ≤96 hrs (≤24 hrs optimal), store at 4°C or room temperature. Specimens received beyond 96 hrs of collection will be processed but results may be compromised because of the prolonged transit time (a comment to this effect will be added to all negative reports). Ensure samples that are being sent to a referral laboratory are packaged in accordance with Transport of Dangerous Goods recommendations for diagnostic samples. |
