Clinical Information:
Description:  Sulfonylurea screening is used in the evaluation of suspected insulinoma and in differentiation of cases of hyperinsulinemic hypoglycemia of unknown etiology. This test is not intended for therapeutic drug monitoring.


Test Indications: This test is only indicated during or shortly after confirmed episodes of hypoglycemia with increased insulin secretion.  Drugs detected by the assay include common first- and second-generation sulfonylureas:

 

First-generation: Chlorpropamide, Tolazamide, Tolbutamide

 

Second-generation: Glimepiride, Glyburide, Gliclazide

THIS TEST IS SOLELY INTENDED FOR CLINICAL USE AND ONLY AVAILABLE TO ENDOCRINOLOGISTS.

 

Please indicate the hypoglycemic agent(s) of interest on the request.

Collection Devices:
Serum (NO GEL) is the preferred specimen (4 mL minimum). Serum collected in gel separator tubes will be rejected.

 

If serum is unavailable, urine (10 mL minimum) may be submitted, and must be collected in Leakbuster™ sterile urine cup with orange lid. Do not collect in same container with other urine tests. No alternative urine containers are acceptable for this test.

Specimen Required:
Serum: 4.0 mL
Pediatric Serum: 4.0 mL

Collection Information: Specimens should be collected during or shortly after an episode of hypoglycemia (plasma glucose <4.0 mmol/L) in order for results to be clinically informative.

Special Processing: 

Serum: Centrifuge and aliquot serum immediately upon receipt. Keep refrigerated.

Urine: Do not aliquot urine specimens. Do not order other urine tests on specimen. Keep refrigerated.

Specimen Stability:

Refrigerated: 7 days (serum and urine)

Frozen: 28 days (serum only)

Shipping & Storage: Ship refrigerated to HSC for send-out to the referral laboratory.

Interpretation & Assay Interferences: Presence of drug(s) indicates that the patient has recently ingested a hypoglycemic agent.

 

Test performance is optimal with serum. Results from urine specimens will reported with comment regarding less than ideal test conditions.
