Test Indications: Secondary screening for systemic lupus erythematosus (SLE) based on ANA results.  Also useful for monitoring SLE disease activity.

Recommendations:
DO NOT order dsDNA as a routine screen with ANA.

A negative result does not necessarily exclude autoimmune disease.  Positive results should not be interpreted in isolation and must be correlated with relevant clinical findings. 

Method of Analysis: The ELISA test kit provides a quantitative in vitro assay for human autoantibodies of the IgG class against double-stranded DNA (dsDNA) in serum.

dsDNA is automatically reflexed for Homogeneous and Speckled ANA patterns with titres ≥160.
dsDNA is also orderable as a stand-alone test for monitoring SLE disease activity. Increasing antibody levels may be associated with flares while decline or negative results may indicate response to treatment or disease remission.
 
The sensitivity in clinically characterized patients with SLE was 60%.  The specificity in a control panel of patients with other autoimmune diseases and health blood donors amounted to 99%.
